What is Palliative Care?

Palliative Care, not to be confused with Hospice/end of life care, aims to assist with managing difficult-to-control symptoms, that may include pain, nausea, anxiety, and stress that an individual dealing with a chronic or life-threatening illness may experience in conjunction with the traditional medical care the patient may be receiving.

Palliative Care supplements medical treatment and if needed, allows for a smooth transition between home health and hospice services when goals of care may have changed.

Our Palliative Care is a specialty program structured under the umbrella of our Home Health Services. This provides patients and families who qualify for this additional support the excellent services available through home health in addition to an experienced palliative care nurse and social worker who work alongside the care team with the goal of enhancing quality of life and outcomes through the individualized Plan of Care.

This team of experts - from multiple medical specialties, including the patient’s PCP, and/or specialists, if any - work together to address the complex needs of the patient and his or her family members. They collaborate to develop and execute a comprehensive care plan aimed to treat the whole individual by offering medical, emotional, and spiritual support.
